StayWell Health Management Program
The StayWell Health Assessment is available to help make positive behavior changes towards a healthier lifestyle. Beginning January 16, 2012, UC will provide a $100 gift certificate to each eligible* employee and retiree and a $50 gift certificate to eligible* spouses/domestic partners who complete the two-step process:
- StayWell Health Assessment (HA) - complete by June 15 , 2012.
- StayWell follow-up activity - complete by December 15, 2012. Follow up programs are dependent upon HA results. See below or click here more details.
Participation in the May 23rd UC Walks event will count as an alternative follow-up program. See flyer for more details.

StayWell Two-Step Program
Once your have completed the Health Assessment (step one), you will need to complete the second step -- the follow-up activity -- to be eligible to receive your incentive award. Your HA results will determine the appropriate follow-up action and you will be guided to either enroll in the NextSteps Wellness Coaching or to complete the 6 week on-line Time Out For Your Health campaign. StayWell NextSteps Wellness Coaching
StayWell's NextSteps coaching is a behavior-based program designed to help you set and meet your goals. There are three ways to participate: telephone with a wellness coach, mail or online. Wellness coaching can take 3-6 months to complete. The recommended deadline to register to have enough time to complete a program is July 31, 2012 as the NextSteps program may take 4 months or more to complete.

*Kaiser members and members of certain bargaining units are not eligible for the StayWell program.
**Employees and their spouses/domestic partners must be enrolled in an eligible UC medical plan on or before January 1, 2011 to be eligible for the incentive.

Gift Card Incentive
You will receive the award confirmation notice a week following completion of your follow-up program. If you provided an email address, the confirmation will be sent to you by StayWell Health Management and may be identified as SPAM. Please check your SPAM mail periodically. If you have questions or need assistance, please call the StayWell Customer Service line at 1-800-721-2693. If you did not provide an email address, the award confirmation will be mailed to the address StayWell has on file. Follow the instructions on the award confirmation to select your gift certificate. You should receive your gift certificate within 10-14 business days.
